Service Caravan continues in Brgy. Magugpo East
The City Government of Tagum conducted the “Project CO - Caring for Others: Service Caravan for Tagumenyos”, a caravan of services wherein basic social services of various LGU offices encompassing health, social welfare, employment, legal consultations, civil registration, agriculture, veterinary, skills training and among others, were delivered closer to Tagumenyos.
During the caravan, the City Civil Registrar’s Office (CCRO) conducted Mobile Civil Registration (Birth Registration Assistance Project) and Maling Dokumento Mo, I-Wasto Ko (MDMIK) Program wherein a total of 76 civil registry transactions were accommodated.
Among the services availed during the said activity were the following:
Late Registration of Birth;
Request for PSA Copy of Birth, Marriage, and Death Certificates;
Correction of Clerical Error and Change of First Name (RA 9048);
Legitimation (RA 9255); and
Affidavit to Use the Surname of the Father (RA 9255).
In this activity, the CCRO facilitated an “on-site” preparation of drafts for Birth Certificate and Affidavits using its latest digital innovation, “Tagum City eMobile Civil Registration System”, which automated, simplified, and standardized the procedures for the Late Registration of Birth by providing printed checklists and instructions to be undertaken by the registrants in conformity to the recent revision of guidelines.
The Office also facilitated a free picture-taking for the applicants for late registration of birth in compliance to the PSA Memorandum Circular No. 2024-17, which requires the said registrants to submit 2x2 picture for their application.
To cater the clients’ applications for Philippine Identification System (PhilSys) Registration, and requests for PSA copies of the Birth Certificates and printed ePhilIDs, the City Civil Registrar’s Office (CCRO) invited the Philippine Statistics Authority Davao del Norte Provincial Statistical Office to facilitate the said services.
